¡Salud! Combined Prior Authorization Request Form
- The three New Mexico ¡Salud!
managed care organizations have agreed upon a common prior authorization
request form. For now, this form must be downloaded, printed, completed
and faxed to the appropriate MCO. NMMS has requested that the MCOs
also accept this information electronically. If one or more MCOs agree, we
will post an online version of this form here as well. The form is
an MS Word document, saved in a file format which will work on all
versions from Win95 onwards. Click here to download

The ¡Salud! Comparative Formulary is unfortunately no
longer available online in regular Web format. It can be downloaded as an
Adobe Acrobat (PDF) file*. This may take up to

- Research Association of Medical and Biological Organizations.
An ad hoc group representing various New Mexico organizations
working to develop a model for sharing data between diverse groups
that would address a community, regional, or state health problem.
